Below are common forms and where to locate them. 

Registration & Enrollment

  • Add Form: For registration errors such as Instructor Permission, Classification, or Time Conflict, please contact the instructor, who can submit an override. Overrides are visible in “Check Your Registration Status” in myOswego.
    • For Independent Study / Research / Teaching Assistantship – Contact the course instructor to submit details online.
    • For prerequisite errors – Contact your advisor, who may submit a Prerequisite Deviation.
    • Late add requests after the official period are subject to a fee.
  • Application for Readmission (Undergraduate)
  • Audit Form: Instructors may request audit status during the Add period.
  • College Withdrawal Form: See FAQs for steps to withdraw from all courses.
  • Course Withdrawal Form: Found in myOswego (Fall/Spring only) under the Registration tab. Requires advisor consultation and may impact financial aid. See calendar for deadlines.
  • Combined Enrollment Form: For undergraduates requesting to take graduate-level courses.
  • E to F Grade Request: May be used once by undergraduates with prior major coursework. Discuss with your advisor.

Academic Programs & Degree Requirements

  • Change of Major/Minor Form: Submit in myOswego (Student Records tab). Discuss changes with your academic department. New students should contact Admissions.
  • Prerequisite Deviation Form: Request through your advisor. Review policy.
  • Program Deviation Form: Contact your advisor to request requirement waivers or substitutions.
  • Replacement/Duplicate Diploma: Includes Apostille-certified copies.
  • Senior Check Forms: Accessible in Degree Works after applying to graduate.
  • Transcript Requests: For overseas use, refer to the Apostille process.

Special Requests

  • Off Campus Study Approval Form: Required for taking courses at another institution for Oswego credit.
  • Overload Approval Form: Contact your advisor. Summer/Winter overloads need Dean approval. Non-degree students contact Extended Learning.
  • Pass/Fail Option Form: Available to juniors, seniors, and non-degree students during the Add period.
    • Limit: One course per term, maximum of four courses (12 credits) overall.
    • Must use Oswego email and receive advisor approval.
